Association for Transpersonal Psychology & Hypnotherapy (ATPH) Application Form

• Associate Membership has no certification requirements. Professional Membership requires 150 hours of more of training.
• Dues are $25 per year and membership is valid from June 1 until May 31 of the next year.
• Upon reviewing & approving your application we will send you your Professional or Associate certificate and the application for Professional Liability Insurance (for Professionals). You need the certificate number when applying for insurance.
